Your privacy matters

Assessment responses can contain sensitive personal information, so this one is built not to collect any.

  • These assessments are completely anonymous. No account is required, and you are never asked for your name, email address or phone number.
  • Your answers and results are held in your own browser only, for this visit. They are never transmitted to MindMonks, never stored on any server, and never shared with any third party.
  • Nothing is attached to a patient record. Nothing appears in a web address, a WhatsApp message, or any analytics or tracking tool.
  • Closing the tab permanently clears all data. You can also clear it yourself at any time using the button on the summary screen.
  • Because no personal data is collected or stored, the Digital Personal Data Protection Act 2023 obligations around consent, retention and deletion do not arise for this tool.

If you decide to book a consultation afterwards, the booking form starts completely blank. Your assessment answers are never carried across. You choose what to share with your clinician.

About these assessments

MindMonks self-assessments use internationally validated screening questionnaires to help you better understand your current symptoms and wellbeing.

These tools are permitted for educational and self-awareness use under their respective licensing terms. PHQ-9 and GAD-7 were developed with an educational grant and are free to use without permission. The WHO-5 is reproduced with attribution to the WHO Regional Office for Europe.

Scores should be interpreted in context. They do not constitute a medical diagnosis, treatment recommendation, or clinical opinion from MindMonks or any of its practitioners.

Results may be influenced by temporary stress, physical-health conditions, medications, sleep, substance use, recent events, and other factors that a validated questionnaire cannot account for.

This screening tool is not a substitute for a comprehensive assessment by a registered psychiatrist, psychologist, or other qualified mental-health professional.

Screening is not diagnosis

What a questionnaire can and cannot tell you

A screening questionnaire asks how often certain difficulties have troubled you over a set period, and adds the answers up. That is genuinely useful — it gives a shared vocabulary and a starting point, and it can make something vague feel describable.

What it cannot do is tell you why. The same answers can come from a difficult few weeks, a thyroid problem, a medication, months of poor sleep, grief, or a condition that would benefit from treatment. Separating those is what an assessment with a clinician is for.

So read a result as a prompt, not a verdict. It is information about the last two weeks, not a label for you.
